I think you have the extensions hidden in your OS. If you're using Windows, and Outerra is correctly installed, then you just have to double click on it, and the add-on will be automatically installed for you. For instance, the file can be opened with almost any archive manager (WinRar, 7Zip, etc...). At this moment the earth is empty, but they will be integrating OSM for roads/buildings placement, and eventually a city generator...The A380 is just a mod/add-on I made, nothing official.
