You'd need to diagnose that inflight. Don't assume it's a bug or sim feature. Quite common to have nav downgrades when flying. Have a look at all the position outputs available in the FMS and try to work out what has happened.

Thank you. Once I figure it out is there a way to force some nav (IRS or GPS) to be the main source and let the others align themselves to it during flight? Or by picking up nearby VORs can I reset the position?

The system will automatically select the best source of position information. Have a look at the POS SUMMARY MFD DATA page, then the FMC Position study window to help understand how it's all working.

Yes, it is understood that the system will pick the best available position source. Nevertheless, how is IRS-FMS disagree fixed in flight? Can an IRS-FMS-disagree be fixed in flight at all?
Unfortunately, there is no QRH info on that one available to me.

Is it just disabling the IRS'es, and re-enabling them in flight, and waiting for a realign?

If you fly in the Middle East with a lot of GPS jamming and spoofing, having an unreliable IRS all of a sudden is not a good thing ...
